Job summary

FutureCare is seeking a licensed Physical Therapist to evaluate and treat movement dysfunction for residents in our Baltimore-area facilities. You will assess, diagnose, and implement individualized plans to restore mobility and independence, coordinating with nursing staff and families.

The role emphasizes compassionate patient care, adherence to guidelines, and ongoing documentation; a Bachelor of Science in Physical Therapy and Maryland licensure are required, with LTC experience preferred.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Maryland licensure or eligibility required.
• Minimum 1 year long-term care experience preferred.
• Competency in identified areas of specialization; understanding accreditation standards.

Responsibilities

• Review resident information and assess impact on treatment.
• Conduct functional assessment and document results.
• Develop and implement individualized treatment plans with achievable goals.
• Supervise and train aides; document care and outcomes.
• Document treatment course and discharge summary clearly.
• Educate residents and families on exercise programs and procedures.
