FutureCare in Maryland is seeking a licensed Physical Therapist to evaluate, diagnose and treat movement dysfunction. You will develop individualized plans to restore mobility and independence, documenting progress and coordinating with nursing staff and families.

Competenze

• Bachelor of Science in Physical Therapy is required.
• Current Maryland license or eligibility is required.
• 1 year of Long Term Care experience preferred.
• Certification in identified areas of specialization.
• Competency in applying physical therapy principles and equipment.
• Ability to assess resident needs and develop a comprehensive care plan.
• Knowledge of accreditation standards and compliance requirements.

Mansioni

• Review resident information and assess impact on treatment.
• Perform a thorough functional assessment using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Document evaluation, treatment, and follow guidelines per policy and regulations.
• Complete re-evaluations and supervise staff as needed.
• Instruct and supervise rehab staff during procedures as appropriate.
• Document in the resident’s medical record after each session.
• Create discharge summaries and orient residents to services.
• Assist in referrals post-discharge and know community resources.
