FutureCare is seeking a licensed Physical Therapist in Maryland to evaluate and treat movement dysfunction, helping residents regain mobility and independence. You will document assessments, develop personalized treatment plans, and supervise rehab staff to ensure quality care.

The role emphasizes ongoing compliance with regulatory guidelines, thorough documentation, and collaboration with nursing and families to maximize residents’ functional abilities.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Current license or eligibility for Maryland licensure expected.
• Minimum 1 year Long Term Care experience preferred.
• Certifications and demonstrated competency in identified areas of specialization.

Responsabilités

• Review resident information and assess impact on treatment plan.
• Conduct thorough functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Document evaluation and treatment per facility policy and regulations.
• Supervise assistants and rehab techs as appropriate during therapy sessions.
• Maintain clear, concise discharge summaries and ensure proper documentation.
