Job summary

FutureCare Health & Management is seeking a Licensed Physical Therapist to evaluate, diagnose and treat movement dysfunction in residents of our Maryland facilities. The role focuses on planning mobility improvements toward independence, supported by a comprehensive treatment program and close collaboration with nursing staff and families.

The position requires a BS in Physical Therapy and Maryland licensure eligibility, with 1 year of long-term care experience preferred.

Qualifications

• Minimum of Bachelor of Science degree in Physical Therapy.
• Current license or eligibility for licensure in the state of Maryland.
• 1 year of experience in Long Term Care preferred but not required; new graduates welcomed.

Responsibilities

• Review resident information and assess impact on treatment.
• Conduct functional assessments and document findings.
• Develop and implement individualized treatment plans with measurable goals.
• Supervise assistants and rehab techs when needed.
• Document treatment course and discharge summaries clearly.
• Educate residents and families on exercise programs and procedures.
