Job summary

FutureCare Associates, Inc. is seeking a Physical Therapist to evaluate, diagnose and treat movement dysfunction for residents in a long-term care setting. You will develop personalized treatment plans, conduct functional assessments and work with nursing staff to maximize residents’ functional abilities.

Qualifications

• Bachelor of Science in Physical Therapy is required.
• Current Maryland license or eligibility for licensure is required.
• Minimum 1 year of long-term care experience preferred.
• Certification and demonstrated competency in identified areas of specialization.
• Ability to assess resident needs and develop a comprehensive plan of care.
• Knowledge of accreditation standards and compliance requirements.

Responsibilities

• Review resident information and assess impact on the treatment plan.
• Conduct and document thorough functional assessments.
• Develop and implement individualized treatment plans with achievable goals.
• Complete documentation following state and facility guidelines.
• Provide supervision to assistants and rehab techs when needed.
• Document treatment courses and outcomes in the resident's medical record.
• Re-evaluate progress and revise plans as required by regulations.
• Educate residents and families on exercises and procedures for post-discharge continuation.
