Job summary

Community Living Centers, Inc. seeks a Medical Coordinator to organize and monitor resident medications, maintain medical records, schedule appointments, and transport residents to appointments within a Residential Group Home setting.

The role requires hands-on care, strong communication with physicians and staff, and adherence to person-centered plans. Training, scheduling flexibility for evenings/weekends, and a focus on safe, supportive care are essential.

Qualifications

• High School Diploma or GED.
• 2-3 years supervisory or Medical Coordinator experience in a Group Home.
• 2 years’ experience working in the mental health field.
• Completion of CMH certifications and training for Oakland, Macomb and Wayne counties in Michigan.
• Able to perform all DSP/DCW duties as needed.
• Must maintain a valid and current driver's license.

Responsibilities

• Maintain close contact with residents regarding medical needs and medications; coordinate with physicians, dentists, psychiatrists, etc.
• Attend all medical appointments with residents and coordinate scheduling.
• Check in all medications and ensure sheets match prescriptions.
• Be on call 24 hours for emergencies; work evenings and weekends as scheduled.
• Attend Person Centered Planning meetings and report health status.
• Communicate with staff and outside agencies regarding health status.
• Provide hands-on care including mobility, bathing, hygiene, and meals.
