Job summary

ACLD in Nesconset, NY is seeking a Medical Coordination Specialist to provide medical coordination and support for people with intellectual and developmental disabilities at a residential site. You will advocate for residents, promote health and safety, and mentor in independent living skills as outlined in each Life Plan.

Responsibilities include overseeing medication administration, coordinating weekly nursing reviews, and arranging medical appointments with families and nurses.

Qualifications

  • High School Diploma or equivalent required.
  • Ability to communicate and understand in English; both verbally and in writing.
  • The ability to obtain and maintain AMAP (Approved Medication Administration Personnel) Certification.
  • The ability to obtain and maintain SCIP Core Certification, CPR, First Aid and AED Certification.
  • One (1) year experience with developmentally disabled population preferred.
  • The ability to assist with lifting and transferring a person; lift up to 15 lbs.
  • The ability to reach, bend, twist, kneel, and squat as you assist people with physical activities.
  • The ability to perform SCIP maneuvers which may require bending, twisting, kneeling, crouching, lying on the ground, and supporting another person’s body weight.

Responsibilities

  • Oversee the dispensing and documentation of medication in accordance with AMAP procedures and follow all AMAP guidelines.
  • Maintain appropriate stock of medications for each person supported.
  • Meet with the residential nurse to conduct weekly nursing coordination.
  • Schedule all medical appointment follow ups as directed.
  • Attend medical appointments with people supported; collect paperwork and submit to nurse.
  • Communicate with families regarding medical appointments as needed.
  • Prepare the MARs on a monthly basis.
  • Review all MARs and documents for accuracy.
  • Ensure refills of medications are ordered in a timely manner.
  • Ensure new prescriptions are obtained in a timely manner as per procedure.
  • Ensure all documents accompany person supported on medical appointments.
  • Complete medication administration error reports as needed.
  • Respect the human dignity and uniqueness of the people you support.
