Cogir Senior Living seeks a dedicated Resident Care Coordinator to bridge residents, families, and care teams in our assisted living communities. You will support scheduling, supervise staff, and ensure care plans meet regulatory standards, while delivering compassionate service.

This role emphasizes medication administration, teamwork, and strong leadership, with weekend rotation required. A valid state medication administration license and First Aid/CPR are essential; related certifications

Qualifikationen

• High School Diploma or equivalent.
• Must have a valid state medication administration license (CNA/HCA) and meet state requirements for medication administration.
• Nurse Delegation and Diabetes Specialty Certificates, or willingness to obtain within a prescribed period.
• An LPN nursing license is a plus.
• Current First Aid and CPR license or ability to obtain.

Aufgaben

• Schedule and assign caregiving and medication tech shifts to ensure adequate coverage and policy compliance.
• Participate in recruitment and interview process for care team members.
• Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
• Oversee the medication room and orders, collaborating with technicians and caregivers.
• Maintain accurate resident records, care plans, incident reports, and medical documentation.
• Coordinate care planning with home health agencies on-site, per policies.
• Coordinate admissions with the sales team, including supervising move-ins.
• Conduct family/resident admission interviews.
• Facilitate communication among residents, families, and healthcare providers to ensure seamless care.
• Address residents' special needs, requests, and complaints.
• Assist with medication administration and provide direct resident care when needed.
• Comply with community and state care regulations.
