Job summary

Sandridge Post Acute is seeking an Activity Assistant to help plan and run a creative activities program for residents. You will support the Activity Director and work with staff to ensure activities meet residents’ physical, mental and psychosocial needs.

The role involves coordinating monthly calendars, documenting attendance, assisting with resident assessments and care plans, and arranging transportation for outings or appointments. This is an on-site position in a long-term care setting.

Qualifications

• High school diploma or equivalent is required.
• Preferably one year of experience in a long-term care facility.
• Ability to read and comprehend policy and procedure materials and communicate effectively.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist with monthly activity calendars and attendance records.
• Communicate with staff, residents and families to support needs.
• Assist with assessments, care plans and progress notes as assigned.
• Arrange transportation for residents when needed and maintain a safe environment.
