Responsibilities

• Assist in planning and conducting activities for residents.
• Develop monthly activity calendar and maintain attendance records.
• Coordinate transportation for residents for outings and appointments.
• Assist in discharge planning and activity care plans.
• Support communication among staff, residents, families, and agencies.
• Keep the Activity Department clean and orderly.
