Job summary

Sandstone Post Acute is seeking an Activities Assistant to support memory care residents with engaging daily activities. You will help create and post monthly calendars in large print and ensure copies are placed in residents' rooms.

You may interview residents or families for preferences and assist in transporting residents to activities. You will document participation, monitor resident responses, and report mood or involvement changes to the Activities Director.

Qualifications

• High school diploma or equivalent preferred.
• Effective verbal and written communication skills.
• Ability to maintain composure in stressful situations.
• Basic computer skills.

Responsibilities

• Assist Memory Care Director in creating a monthly activities calendar.
• Ensure calendars are posted in prominent locations visible to residents and visitors.
• Interview residents or families to obtain activity preferences.
• Help residents access activity locations within the facility.
• Conduct activities to promote residents' physical, social, and mental well-being.
• Document attendance and describe resident responses in progress notes.
• Observe mood and involvement; report changes to the Activities Director.
• Involve residents and families in planning activities when possible.
• Coordinate for offsite field trips and transportation as needed.
• Coordinate needs with other departments to avoid interruptions.
• Assist with Resident/Family Council meetings as directed.
