Job summary

San Diego Post Acute invites an Activities Assistant to join our healthcare team in a full-time role. You will help plan and conduct activities to enrich residents' lives and support quality of care.

You will work with staff, families, and residents to implement monthly calendars, coordinate transportation, and assist in assessments and discharge planning. A high school diploma and prior LTC experience are preferred.

Qualifications

• High school diploma or equivalent; one year in long term care facility preferred.
• Ability to read technical procedures and policy manuals.
• Effective communication with residents, families, and staff.
• Basic arithmetic for activities logistics.

Responsibilities

• Plan and conduct activities for residents, individual and group.
• Assist in creating monthly activity calendars and track attendance.
• Coordinate transportation for outings and appointments.
• Assist in discharge planning and care plan development.
• Maintain a clean, organized, and secure Activity Department.
