Job summary

Cedar Creek Post Acute is seeking an Activity Assistant to support the Activity Director in delivering a creative and interactive activities program for residents. You will help plan, conduct and document a range of activities designed to meet physical, mental and psychosocial needs while following applicable standards and guidelines.

You will work with staff across departments, coordinate monthly calendars, assist with assessments, and help arrange transportation for residents as needed.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures and policy manuals.
• Ability to present information clearly to managers and staff.

Responsibilities

• Participate in planning and conducting individual, small and large group activities.
• Assist in communication across staff, residents, families and agencies.
• Contribute to community planning related to resident services.
• Develop and maintain monthly activity calendar and attendance records.
• Assist QA/Assessment Committee with action plans to address deficiencies.
• Participate in discharge planning and activity care plans.
• Arrange transportation for residents for outings or appointments.
• Create monthly activity schedules including outings and in-room activities.
• Encourage resident participation in hobbies, crafts and reading.
