Job summary

EAH Housing seeks a Resident Health Services Coordinator to assist residents in achieving housing stability and better health outcomes. You will provide health navigation, education, and help connect residents to Medi-Cal benefits and primary care across our California communities.

The role requires collaboration with MCPs, CHW supervisors, and service partners, plus ongoing outreach, program development, and documentation.

Qualifications

• Bachelor’s degree in social services, public health, or related field.
• 3–5 years in human services, community-based work, CHW roles.
• Two+ years with supportive housing or social services.
• Must have valid driver’s license and DMV clearance.
• Lived experience with the served communities preferred.

Responsibilities

• Coordinate non-clinical, health-related support and health education.
• Assist residents in understanding and accessing Medi-Cal and primary care.
• Support CalAIM Community Supports or ECM and coordinate with MCPs.
• Help residents address social drivers of health and housing stability.
• Develop monthly newsletters and calendar of events.
• Maintain resident engagement documentation and reports.
