Job summary

Christian Community Health Center seeks a Resident Service Coordinator to join the supportive services team. You will help residents achieve health, well‑being, and financial goals through intake, resource coordination, case management, and data‑driven program decisions.

Responsibilities include care coordination, referral services, life skills workshops, and monthly events. You will maintain confidential records and contribute to quality initiatives across staff meetings.

Qualifications

• Bachelor’s degree in Social Work or related field; 3–5 years of related experience.
• Associates degree with at least five years of related experience.
• Valid Driver’s License and vehicle insurance required.
• Occasional late or weekend hours; exceptional oral and written communication skills; ability to relate and work with diverse populations.
• Some computer skills required.

Responsibilities

• Deliver care coordination: assessment, advocacy, planning, facilitation, monitoring, and evaluation of options to meet residents' needs.
• Care coordination and referral services for health, emotional needs, employment, and other resources.
• Lead workshops on life skills, employment readiness, benefits, financial literacy, health and wellness.
• Plan and coordinate monthly events.
• Participate in monthly meetings with Property Manager and staff to improve service delivery.
• Maintain resident service documentation with confidentiality.
• Conduct satisfaction surveys and present data for improvements.
• Prepare and distribute a monthly calendar of events for residents.
• Submit monthly service reports to direct supervisor.
• Participate in supervision and quality initiatives.
• Perform other related duties as assigned.
