Job summary

FutureCare seeks a Physical Therapist to evaluate, diagnose and treat movement dysfunction, restoring mobility and independence for residents. The role emphasizes compassionate care and comprehensive evaluation within a Maryland facility.

You will collaborate with nursing staff and families, document sessions, supervise assistants, and contribute to discharge planning while adhering to regulatory standards and facility policies.

Qualifications

• Bachelor of Science in Physical Therapy.
• Current Maryland license or eligibility for licensure.
• Minimum of 1 year of Long Term Care experience preferred.
• Certification and demonstrated competency in identified areas of specialization.
• Ability to assess resident needs and develop a comprehensive plan of care.

Responsibilities

• Review resident information and assess impact on treatment.
• Conduct thorough functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Complete documentation and re-evaluations per policy and state/federal regulations.
• Orient residents to services and inform of potential risks during procedures.
• Work with nursing staff and families to maximize function.
• Instruct residents and families on exercise programs to continue post-discharge.
