Job summary

FutureCare in Maryland seeks a Physical Therapist to evaluate movement dysfunction, diagnose and treat mobility issues, and create personalized plans to restore independence. You will document progress, supervise rehab staff, and coordinate with nursing and families for best patient outcomes.

Qualifications

• Bachelor of Science in Physical Therapy and Maryland license eligibility.
• 1 year of Long Term Care experience preferred but not required.
• Competent in applying PT principles and documentation.

Responsibilities

• Review resident information and assess impact on treatment.
• Conduct functional assessments using standard procedures.
• Develop and implement an individualized treatment plan with achievable goals.
• Supervise aides and rehab staff during therapy sessions.
• Document treatment course and discharge summaries clearly.
• Educate residents and families on exercise programs for continued care.
• Coordinate with nursing staff and community resources as needed.
