Job summary

FutureCare is hiring a Physical Therapist to evaluate and treat movement dysfunction, helping residents regain mobility and independence. You will create individualized plans and work with the care team to maximize functional outcomes in a long-term care setting.

The position requires a Bachelor of Science in Physical Therapy and current Maryland licensure or eligibility, with preference for LTC experience. We offer competitive salary and comprehensive benefits.

Qualifications

• Bachelor of Science degree in Physical Therapy is required.
• Current license or eligibility for Maryland licensure.
• 1 year of long-term care experience preferred but not required.
• Certification and demonstrated competency in identified areas of specialization.
• Ability to assess resident needs and develop and implement a comprehensive plan of care.
• Knowledge of accreditation standards and compliance requirements.

Responsibilities

• Review resident information to assess impact on treatment.
• Conduct and document functional assessments.
• Develop and implement individualized treatment plans with achievable goals.
• Comply with facility policies and regulations.
• Supervise rehab staff and document interventions.
• Document treatment in medical records and discharge summaries.
• Orient residents and families to services and procedures.
• Collaborate with nursing staff to maximize function.
• Educate residents and families on exercise programs.
• Assist in referrals and utilize community resources.
