Job summary

FutureCare is seeking a Physical Therapist to evaluate, diagnose and treat movement dysfunction, and to create plans that restore mobility and independence for residents. The role emphasizes supervision of rehab staff, documentation, and collaboration with nursing and families.

The candidate should hold a Bachelor of Science in Physical Therapy, possess a Maryland license or eligibility, and preferably have 1 year of long-term care experience. A competitive benefits package is offered.

Qualifications

• Must hold a current Maryland physical therapy license or eligibility.
• Minimum of 1 year long-term care experience preferred.
• Demonstrated competency in applying physical therapy principles and techniques.
• Ability to develop a comprehensive plan of care based on resident needs.

Responsibilities

• Review resident information and assess impact on treatment.
• Conduct and document functional assessments.
• Develop and implement individualized treatment plans with achievable goals.
• Complete documentation in the resident’s medical record after each session.
• Supervise assistants and rehab techs when delivering therapy.
• Revise treatment plans as needed throughout care.
• Provide discharge summaries and inform residents about post-treatment procedures.
• Collaborate with nursing and families to maximize functional abilities.
