Job summary

PACS is seeking an Activity Assistant to support an engaging activities program for residents in a long-term care setting. The role ensures activities align with assessments and regulatory standards while addressing the interests and well-being of each resident.

The position emphasizes collaboration with the Activity Director and care team to plan, implement, and monitor monthly schedules, group activities, and resident outings, including accommodations for diverse needs.

Qualifications

• Education and/or Experience: High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.

Responsibilities

• Participate in planning and conducting individual, small and large group activities.
• Assist in providing communication between employees, residents, families and agencies to meet residents' needs.
• Assist in development of monthly activity calendar and maintain attendance records.
• Support discharge planning and development of activity care plans and resident assessments.
• Arrange transportation for residents when necessary for outings, appointments, or discharge.
