Job summary

PACS is seeking an Activity Assistant to help run a creative and interactive activities program for residents in a long-term care setting. You will assist the Activity Director to design and implement activities that meet each resident’s physical, mental, and psychosocial needs, following all applicable standards and regulations.

The role involves planning monthly calendars, coordinating transportation, and promoting resident engagement through hobbies, crafts, and reading.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in communication between staff, residents, families, and the public to meet resident needs.
• Develop monthly activity calendar and maintain attendance records.
• Assist in discharge planning and activity care plans for residents.
• Arrange transportation for residents as needed, including outings and appointments.
• Encourage residents to participate in self-initiated activities (hobbies, crafts, reading).
• Provide materials such as reading materials in Braille or audio books as appropriate.
• Assist with assessment documentation and quarterly progress notes.
• Keep Activity Department clean, orderly and secured.
