The position requires at least a high school diploma, with preferred experience in long-term care. Strong communication and organizational skills are essential to coordinating residents, families, staff, and external agencies.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long-term care facility.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the resident-centered activities for the facility.
• Keep abreast of current federal and state regulations, as well as professional standards.
• Provide good communication between employees, residents, families, and agencies to meet residents' needs.
• Assist in discharge planning, activity care plans, and resident assessments.
• Develop monthly activity schedules including outings, group activities, and in-room activities.
