Job summary

PACS Group is seeking an Activity Assistant to support the Activity Director in delivering a creative, person-centered activities program that complies with all applicable standards and guidelines.

You will help plan and conduct individual and group activities, coordinate calendars, arrange transportation, document progress, and communicate with residents, families, and staff to promote engagement and well-being.

Qualifications

• High school diploma or equivalent is required.
• Preferably one year of experience in a long term care facility.

Responsibilities

• Participate in planning and conducting individual and group activities.
• Assist in communications among staff, residents, families and external parties to support resident needs.
• Assist in development and maintenance of monthly activity calendar and attendance records.
• Assist in arranging transportation for residents to appointments or outings.
• Assist in activity care plans and resident assessments; support discharge planning.
