Job summary

PACS is seeking a Staff Speech-Language Pathologist to evaluate residents, develop individualized treatment plans, and collaborate with families and the health team. The role includes supervision of CFY clinical fellows or students and ensuring compliant documentation.

The candidate will work within post-acute care settings, provide education on goals, home programs, and safety strategies, and participate in care conferences and quality improvement initiatives.

Qualifications

• Bachelor’s degree in Speech-Language Pathology; Masters or Doctorate preferred.
• Proficient in Net Health and Point Click Care.
• Ability to read and comprehend policy and procedure manuals; communicate with managers, coworkers, and families.

Responsibilities

• Evaluate residents to determine speech therapy needs, functional limitations, and rehabilitation potential.
• Develop and update individualized treatment plans to improve communication and swallowing outcomes.
• Supervise CFY Speech Clinical Fellows and students as applicable.
• Communicate progress and discharge plans with physicians, nursing staff, residents and families.
• Document accurately and timely per facility, government, and payer requirements.
