Job summary

(Cascade Terrace SNF Healthcare, LLC) is seeking a Staff Speech-Language Pathologist to evaluate and treat residents, develop individualized plans, and document outcomes in medical records. You will train staff, supervise CFY fellows, and communicate progress with families, physicians, and the care team.

Responsibilities include routine assessments, discharge planning, infection-control compliant practice, and contributing to quality improvement initiatives within the facility.

Qualifications

• Education and/or Experience: Bachelor’s degree in Speech-Language Pathology; Masters or Doctorate preferred.
• Proficient in computer systems for documentation and data entry (Net Health, Point Click Care).
• Ability to read and interpret policy manuals and communicate with diverse stakeholders.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise CFY Speech Clinical Fellows and students as applicable.
• Communicate progress with physicians, nursing staff, residents and families.
• Complete timely, accurate documentation in accordance with regulations.
• Contribute to quality improvement initiatives and facility rehab program development.
