Job summary

Qualifications

• Education and Experience: Associate degree or two years of college in Social Work or related field, and two years’ case management experience.
• Knowledge of HMIS databases and client records management.
• Strong communication, organization, and problem-solving skills.
• Ability to work with diverse populations.
• Ability to document and report progress accurately.
• Ability to interview clients and assess eligibility.

Responsibilities

• Interview applicants to determine eligibility and establish program goals.
• Develop client intake and orient them to required forms.
• Monitor program compliance and support case workers.
• Provide crisis intervention counseling as needed.
• Maintain HMIS data accuracy and client records.
