Job summary

Hireology is seeking a Social Worker to provide compassionate care and support for residents and families in a long-term care setting. The role requires reliable attendance, strong communication, and collaborative teamwork to address social and emotional needs.

You will develop social histories, create individualized care plans, and participate in interdisciplinary conferences while coordinating discharge planning and staff training to ensure holistic well-being.

Qualifications

• Bachelor’s Degree in Social Work (BSW) or Social Worker Designee certification.
• Strong communication and interpersonal abilities.
• Experience in long-term care settings preferred.

Responsibilities

• Develop detailed social histories and assessments for residents.
• Create individualized care plans to support resident goals and well‑being.
• Participate in interdisciplinary care plan conferences and contribute to care planning.
• Provide individual and group support to residents and families during crises or transitions.
• Assist with new resident orientation and help families adjust to the facility.
• Coordinate discharge planning for smooth resident transitions.
• Facilitate staff in-service trainings and educational programs in social services.
• Build relationships with hospitals, doctors, churches, and community agencies for referrals and ongoing care.
