Job summary

Hireology seeks a dedicated Social Worker to support residents' social and emotional well‑being in a long‑term care setting. Reliability, trustworthiness, and attendance consistency are essential, with multitasking in a dynamic environment and collaboration with staff.

The role focuses on developing social histories, care plans, crisis support, orientation for new residents, and discharge coordination, while partnering with hospitals and community agencies.

Qualifications

• Bachelor's Degree in Social Work (BSW) or Social Worker Designee certification.
• Effective communication and interpersonal skills.
• Experience in long-term care settings.
• Compassionate, organized and resident-focused.

Responsibilities

• Develop detailed social histories and assessments for residents and identify social and emotional needs.
• Create individualized care plans supporting resident goals and emotional well-being.
• Participate in interdisciplinary care plan conferences and contribute to care planning.
• Provide individual and group support to residents and families during crises or transitions.
• Assist with new resident orientation and help residents and families adjust to the facility.
• Coordinate discharge planning for smooth resident transitions.
• Facilitate staff in-service training and educational programs related to social services.
• Build relationships with external partners to support referrals and ongoing care.

Education
