Qualifications

• Bachelor's degree in Social Work or equivalent social work certification.

• Experience in long-term care settings preferred.

Responsibilities

• Develop social histories and assessments for residents.
• Create individualized care plans supporting resident goals.
• Participate in interdisciplinary care planning conferences.
