The position focuses on social histories, individualized care plans, crisis support, and discharge planning. Strong communication and collaboration with families and external partners are essential in this role.

Qualifications

• Bachelor’s Degree in Social Work (BSW) or certification as Social Worker Designee.
• Strong communication and interpersonal skills.
• Experience in long-term care settings preferred.
• Compassionate, organized, and committed to residents’ holistic well‑being.

Responsabilités

• Develop detailed social histories and assessments for residents and identify social and emotional needs.
• Create individualized care plans to support resident goals and wellbeing.
• Participate in interdisciplinary care plan conferences and contribute to care planning.
• Provide individual and group support to residents and families during crises or transitions.
• Assist with new resident orientation and adjust residents and families to the facility.
• Coordinate discharge planning for smooth transitions between settings.
• Facilitate staff in-service training and related educational programs.
