Responsibilities

• Develop detailed social histories and assessments for residents and identify related needs.
• Create individualized care plans supporting resident goals and well-being.
• Participate in interdisciplinary care conferences and care planning.

• Assist with new resident orientation and adjust to facility life.
• Coordinate discharge planning for smooth transitions.
