Job summary

KORU HEALTH LLC in Mequon, Wisconsin seeks a Services Coordinator to support department heads and staff in delivering resident-directed services. Under the Director of Clinical Services, ensure high-quality care aligned with resident needs, regulations, and internal policies.

Responsibilities include supporting nursing assessments, supervising medication documentation, coordinating service plans, and fostering quality improvement across wellness, clinical management, and safety.

Qualifications

• Minimum 3 years of experience in a nursing care community or related healthcare environment.
• Knowledge of DHS Sections 83 and 89 regulations preferred.
• Strong team player with proven leadership and supervisory skills.
• Consistently maintains high levels of resident satisfaction.
• Excellent communication with residents, families, staff, officials, hospitals and the public.
• Excellent customer service with courteous and helpful demeanor.
• Strong problem-solving and ability to develop alternatives.
• Ability to plan, organize, develop, implement and interpret care services programs.

Responsibilities

• Supports nursing staff during medical assessments and ongoing resident evaluations.
• Monitors medication administration, MARs, and documentation; maintains resident records and care plans as directed.
• Coordinates service plans and communicates with physicians or pharmacies regarding orders and changes.
• Provides regular support for resident care needs and follow-up.
• Pursues quality improvement in wellness, clinical management, medication management, risk management, training, safety, and survey readiness.
• Supports hiring, training, and oversight of clinical care staff.
• Ensures compliance with DHS 83 and 89 policies, internal procedures, and reporting requirements.
• Maintains communication with supervisor and other department heads about resident service matters.
• Keeps knowledge current on State DHS 83 and 89 regulations.
• Available for on-call duties as needed.
