Touchmark at The Ranch in Prescott, AZ is seeking a Resident Care Manager to oversee care for residents in Assisted Living and Memory Care, ensuring dignity and high-quality service. This is a salaried, exempt role focused on coordinating care plans, communicating with physicians, and guiding a caregiving team.

Ideal candidates hold a bachelor’s degree in a related field, licensure, and proven experience in licensed care.

Qualifications

• Bachelor’s degree in health administration, social work, psychology, gerontology, or related field.
• Current ALF/RCF Administrators license preferred.
• RN or LPN encouraged to apply.
• Minimum two years of successful experience in licensed care.
• Med aide card and CPR/First Aid certification if needed.

Responsabilités

• Serve as the primary contact for residents or families regarding care; manage reminders and communications with physicians and team members.
• Observe residents, report needs and recommendations to Health Services Director, and ensure care plan completion and documentation.
• Partner with Recruiter and Scheduler to recruit and onboard new caregiving team members.
• Complete quality assurance audits and maintain organized resident charts.
• Work on-site delivering direct resident care during outbreaks and other events.

As a Resident Care Manager, you will ensure residents in Assisted Living and Memory Care receive the highest level of dignity and care. This is a salaried position, and it is exempt from overtime.

Education And Experience Requirements

• Bachelor’s degree in health administration, social work, psychology, gerontology, or related field. Previous social services experience preferred. Current ALF/RCF Administrators license preferred.
• If you are an RN or LPN, you are encouraged to apply for this position!
• Have a minimum of two years of successful experience in licensed care;
• Must have experience as a caregiver and med tech;
• Basic understanding of the conditions and feelings associated with older adults;
• Possess a willingness to continue learning and improving skills, as well as a commitment to confidentiality;
• Must possess a med aide card and be willing to obtain CPR/First Aid certification if needed.

Position Details

• Serve as the primary point of contact for residents or resident families regarding resident care. This includes scheduling and sending resident care plan reminders and effectively communicating with physicians, other health care professionals, discharge planners, family members, and team members.
• Observe residents, determine if needs are addressed, and report observations and recommendations to the Health Services Director. This includes conducting service plan review meetings and making changes, ensuring the care plan is completed, including pre-work and post-work from the meetings as well as all documentation is completed.
• Partner with Recruiter and Scheduler to recruit and onboard new caregiving team members.
• Complete quality assurance audits and ensure resident chart is organized.
• Must be able to work on-site and provide care directly to residents through clinical outbreaks of flu, norovirus, and other diseases, including pandemic situations.

• Demonstrates knowledge of and ability to practice basic principles of coordinating all aspects of care (i.e. care plan, medical appointments, labs, medication, etc.)

• Must possess qualities of leadership, responsibility, time management, compassion, and patience;
• Ability to observe resident behavior and communicate such in an intelligent and professional manner;
• Have a genuine interest in caring for elderly residents and working in a geriatric environment;
• Be able to maintain patient and tactful composure when dealing with residents, family members, staff members, and visitors.
