Frontier Senior Living is seeking a Resident Care Coordinator for the Solace Glen at Cypresswood Memory Care community in Spring, TX. The role focuses on coordinating care plans, supervising caregivers, and ensuring compliance with regulations to deliver compassionate resident care.

The candidate will work closely with the executive director and RN, support staff hiring and training, and manage daily operations including budgeting for the health services department.

Qualifications

• Experience in senior care and supervising staff required.
• Must have CPR certification and prefer English proficiency.
• High school graduate preferred.

Responsibilities

• Coordinate care plan conferences with Residents and family members.
• Maintain caregiver task lists and update ADL changes.
• Schedule service plans per state guidelines and order supplies.
• Communicate with home health, hospice, and ancillary providers; document visits.
• Assist RN with MAR audits, medication room organization, and incident reports.

The Resident Care Coordinator requires an understanding and commitment to the principals and values of the community as well as an ability and willingness to train caregivers in tasks required for delivering quality care to the residents of the community.Primary Duties and Responsibilities:Conducts Coordination of Care plan conferences with each Resident and Responsible family membersMaintains current tasks lists for caregivers, and updates timely as ADL changes occurSchedules the service plan dates according to state regulation guidelinesOrders supplies as well as durable medical equipment as neededAssure home health, hospice and ancillary care providers, communicate their visits and provide necessary documentation of their visit.Completes MAR audits as recommended by RN.Monitors the Medication Room to assure organization and cleanliness.Assists RN with tracking and recording incident reports.
