Job summary

Frontier Senior Living's Solace Glen at Cypresswood Memory Care community in Spring, TX is seeking an outstanding Resident Care Coordinator to ensure high-quality care for residents and oversee daily operations.

The role involves coordinating care plan conferences, supervising caregivers, maintaining task lists, scheduling service plans, and collaborating with health providers while upholding CPR certification and English proficiency.

Qualifications

• Experience in senior care and supervising staff is required.
• High school graduate preferred.
• CPR certification is required.
• Proficiency in English preferred.

Responsibilities

• Coordinate care plan conferences with residents and families.
• Maintain caregiver task lists and update for ADL changes.
• Schedule service plan dates per state guidelines.
• Order supplies and durable medical equipment.
• Coordinate home health, hospice and ancillary providers and document visits.
• Complete MAR audits with RN.
• Monitor the Medication Room for organization and cleanliness.
• Assist RN with incident reporting and investigations.
• Support staff hiring, training, management and counseling.
• FIll in for Receptionist, Medication Aide or Caregiver as needed.
• Manage day-to-day operations as directed by Executive Director.
• Maintain monthly budget for the health services department.
• Other duties as assigned by the Nurse or Executive Director.
