Laurel Glen at Sugar Land Assisted Living & Memory Care seeks a qualified Resident Care Coordinator to join our Sugar Land, TX community. You will coordinate care plans, supervise caregivers, and ensure adherence to regulatory standards while collaborating with the Executive Director and RN.

The role requires training and scheduling caregivers, documenting visits, and supporting the budget for health services.

Qualifications

• Experience coordinating care plans and communicating with residents and families.
• Experience supervising staff in a senior care setting.
• Strong understanding of state regulations and documentation practices.

Responsibilities

• Coordinate Care plan conferences with residents and families and track ADLs.
• Maintain caregiver task lists and update according to changes.
• Schedule service plan dates per state guidelines and manage budget for health services.
• Oversee MAR audits, medication room organization, and incident reporting with RN support.
• Assist with staff hiring, training, and performance counseling; stand-in for reception or meds aide as needed.
• Support Executive Director and RN in daily operations and ensure coverage.

The Resident Care Coordinator requires an understanding and commitment to the principals and values of the community as well as an ability and willingness to train caregivers in tasks required for delivering quality care to the residents of the community.Primary Duties and Responsibilities:Conducts Coordination of Care plan conferences with each Resident and Responsible family membersMaintains current tasks lists for caregivers, and updates timely as ADL changes occurSchedules the service plan dates according to state regulation guidelinesOrders supplies as well as durable medical equipment as neededAssure home health, hospice and ancillary care providers, communicate their visits and provide necessary documentation of their visit.Completes MAR audits as recommended by RN.Monitors the Medication Room to assure organization and cleanliness.Assists RN with tracking and recording incident reports.
