Job summary

Cascade Senior Living Services in Tacoma, WA is seeking an experienced Licensed Practical Nurse (LPN) to join its leadership team and oversee quality care in Memory Care and Behavioral Health communities. You will work with the Director of Resident Services and clinical staff to conduct assessments, guide care planning, and ensure regulatory compliance while preserving residents' dignity and independence.

Responsibilities

• Conduct, review, and document annual nursing assessments for residents.
• Coordinate and participate in annual care conferences with residents, families, and DSHS case workers.
• Review and assist with updating nursing services portions of residents' care plans.
• Provide guidance and leadership in delivery of nursing and personal care services, including staff supervision and training.
• Conduct routine rounds to monitor care quality and staff performance.
• Collaborate with physicians, families, outside agencies and staff to promote continuity and quality of care.
• Respond to incident reports with proper documentation and preventative measures.
• Assist with supply ordering and monitoring operational and clinical trends.
• Monitor key care indicators like showers, vital signs, missed medications, monthly weights and medications.
• Support ongoing quality improvement and regulatory readiness.

Cascade Park Gardens is seeking an experienced, compassionate, and detail-oriented Licensed Practical Nurse (LPN) to join our leadership team and partner with our Director of Resident Services in overseeing & supporting the delivery of quality care within our Memory Care and Behavioral Health community .

This is a highly visible leadership position & y ou will work alongside a supportive clinical team that includes our Director of Resident Services, Staffing Care Coordinator, Floor Nurse, and Med -Techs .

The ideal candidate brings strong geriatric nursing experience, excellent communication skills, sound clinical judgment, and a genuine commitment to preserving the dignity, independence, and quality of life of our residents.

Qualifications & Skills

• C current LPN license or applicable q ualified a ssessor credentials.
• 2-5 years of geriatric nursing experience preferred.

Strong knowledge of nursing care practices, particularly in geriatric and memory care settings.

Knowledge of applicable state Boarding Home regulations an d requirements related to resident care.

Excellent interpersonal, communication, and organizational skills.

Ability to work effectively in a fast-paced, team-oriented environment.

Positive attitude with a high level of energy and professionalism.

Strong attention to detail and ability to manage multiple priorities.

Responsibilities

• Help ensure nursing and personal care services are delivered consistently and according to each resident's individual needs.
• Provide guidance and leadership in the delivery of nursi ng and personal care services, including:

Staff recruitment and supervision, Training and staff development, Performance management and discipline, Resident assessments, Medical records, Regulatory compliance

• Conduct routine rounds throughout the community to observe resident care firsthand and help ensure consistently high standards of service and in the moment training.
• Collaborate effective with physicians, residents, families, outside agencies, consultants, department heads, shift supervisors, and frontline staff to promote continuity and quality of care.
• Review and respond to incident reports, ensuring appropriate documentation , evaluating contributing factors, and implementing preventative measures when appropriate .
• Assist the Director of Resident Services with supply ordering, staff communication, and monitoring operational and clinical trends.
• Assist with tracking and monitoring key care indicators, including:

Showers, Vital signs, Missed medications, Monthly weights, Hazardous drugs, Medication passes, Narcotic records, Incident reports, Annual survey preparation and performance

• Support ongoing quality improvement and regulatory readiness throughout the community.
