Qualifications

• One year direct care experience in long-term care or residential setting preferred.
• Medication Technician Certification to be obtained (training provided).

Responsibilities

• Consult with DSS to complete Auxiliary Grant and Medicaid applications.
• Complete original AG applications if not submitted by hospital.
• Obtain verifications and determine DSS locality for payment.
• Renew annual Auxiliary grants and file with the DSS office.
• Coordinate medical and other appointments and transportation.
• Review medical paperwork and medication changes with providers.
• Complete healthcare reviews and track resident charts.
• Assist residents with financial transactions and documentation.
• Ensure confidentiality of resident information and participate in staff meetings.
