Job summary

Rose Hill Health and Rehab Center in Berryville, VA is seeking a Social Services professional to coordinate resident support and advocacy, ensuring compassionate care and compliant program delivery. You will partner with administration, medical and nursing staff to assess resident needs, plan services, manage discharge planning, and connect residents with Medicare/Medicaid and other community resources.

Responsibilities

• Assist in planning, developing, organizing, implementing and directing of the social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Develop and implement policies identifying medically related social needs.
• Participate in community planning related to facility interests and residents.
• Participate in discharge planning, resident assessments.
• Submit administrative forms and reports to the Director.
• Involve residents and families in planning social service programs.
• Arrange transportation to other facilities when necessary.
• Provide information on Medicare/Medicaid and other financial assistance.
• Prepare discharge plans and present to committees.
• Coordinate social service activities with other departments.
• Work with consultants and implement recommended changes.
• Make routine resident visits and provide services.
• Prepare written and oral reports for the Director.
• Assist in obtaining community resources for residents.
