Great Lakes Management in West Saint Paul, MN is seeking a dedicated Resident Assistant for AM and NOC shifts. You will provide hands-on care and emotional support, assist in dining, and participate in activities while communicating with residents and families and informing the case manager of any changes.

The role offers AM (6am-2:30pm) and NOC (10:30pm-7am) shifts with shift differentials and a 401(k) match.

Qualifications

• Six months of experience in long term care or assisted living preferred.
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant preferred.
• Ability to make responsible choices and decisions in residents' best interests.

Responsabilités

• Assist residents with ADLs as indicated in their service agreement.
• Help residents reach and maintain highest level of independence.
• Provide residents with physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per service agreement.
• Observe clients and report changes in condition to the case manager.
• Interact with residents to provide reassurance, engagement and redirection.
• Complete all required documentation daily and in the care schedule.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident's service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Assistresidents with ADL's (Activities of Daily Living) asindicatedon the individualized service agreement
• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Qualifications :

• Registered as a Nursing Assistant, Home Health Aid or Certified Nursing Assistant (Preferred)
