Job summary

Great Lakes Management (GLM) is seeking a Resident Assistant in West Saint Paul to provide hands-on care, both physical and emotional, per each resident's service agreement. You will communicate with residents, families, and notify the case manager of changes in condition.

The role supports dining, activities, and outings; requires empathy, reliability, and ability to document daily care and observations. Shift options include early morning and overnight positions.

Qualifications

• Six months of experience in long term care or assisted living preferred.
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant preferred.
• Ability to make responsible choices and act in the resident's best interests.

Responsibilities

• Assist residents with ADLs as indicated in the service agreement.
• Help residents reach and maintain highest level of independence.
• Provide residents with physical and emotional support.
• Help with treatments as delegated by the RN per the service agreement.
• Observe clients and report changes to the case manager.
• Interact with residents to provide reassurance, engagement and redirection.
• Complete all required documentation daily and in the schedules/records.

Resident AssistantJob Objective(s):

The Resident Assistantis responsible forprovidinghands-on care, both physical and emotional, as outlined in each resident’s service agreement.Responsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns.The Resident Assistantassistsin the dining room andparticipatingin activities and outings.

Resident AssistantJob Responsibilities:

• Assistresidents with ADL’s (Activities of Daily Living) asindicatedon the individualized service agreement
• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Qualifications:
