Job summary

Great Lakes Management in Minnesota is seeking a Resident Assistant to provide hands-on care and emotional support to residents, following each resident’s service agreement. You will communicate with residents and families and notify the case manager of any changes in condition.

You will assist in dining and activities, supporting residents to maintain independence while adhering to care plans and regulatory guidelines.

Qualifications

• Six months of experience in long term care or assisted living (Preferred).
• Registered as a Nursing Assistant (CNA), Home Health Aide (HHA) or Certified Nursing Assistant (Preferred).
• Ability to make responsible choices and decisions and act in the resident’s best interests.

Responsibilities

• Assist residents with ADLs as indicated in the service agreement.
• Help residents reach and maintain highest level of independence.
• Provide residents with physical and emotional support.
• Assist with treatments as delegated by RN per service agreement.
• Observe clients and report changes to the case manager.
• Interact with residents to provide reassurance, engagement, and redirection.
• Complete daily documentation and schedules.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Assistresidents with ADL’s (Activities of Daily Living) asindicatedon the individualized service agreement
• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.
