Reliant Living Centers of Oklahoma, Inc. is seeking a devoted ICF Case Manager to support individuals with IDD in an ICF/IID setting in Oklahoma City/Del City. You will coordinate services, advocate for resident rights, and facilitate communication with families and providers.

The role emphasizes dignity, independence, and person-centered planning, working with QIDP, nursing, and direct-care staff to ensure goals are achieved and documentation is accurate and confidential.

Qualifications

• Associate degree in psychology or related field is required.
• Bachelor's degree in psychology or related field is preferred.
• CNA or CMA certification is a plus.
• Strong written and verbal communication skills.
• Ability to advocate for residents while respecting choices and rights.
• Familiarity with HIPAA and confidentiality standards.

Responsibilities

• Coordinate IPPs and individual service plans and participate in planning activities.
• Coordinate services with families, guardians, and service providers.
• Assist with scheduling medical appointments and transportation.
• Accompany residents to appointments and community activities as needed.
• Maintain accurate, timely documentation and protect resident confidentiality.
• Support resident rights, dignity, and meaningful community participation.
