Job summary

The Oklahoma Department of Human Services in Shawnee, Oklahoma is seeking a Case Manager III H21C to manage and coordinate support for individuals with intellectual and developmental disabilities. The role includes leading the IDT, developing and monitoring the IP, and serving as the advocate for clients and guardians.

Travel is extensive; you must hold a valid driver's license and maintain required car insurance, with the position based in the state civil service system and offering full state

Qualifications

• Bachelor's degree in a human services field and 3 years of professional experience.
• OR a bachelor’s degree with 3 years in related fields and 1 year in ID/DD; or nursing license with 3 years in ID/DD.

Responsibilities

• Manages and prioritizes a caseload and provides services to individuals with intellectual and/or developmental disabilities and their families.
• Assists the team in identifying community and agency resources to meet individuals’ needs; ensures that necessary services are provided to individuals.
• Serves as the individual’s Qualified Intellectual Disabilities Professional (QIDP); advocates for clients and coordinates with providers.
