Integritus Healthcare is seeking an Admissions Coordinator to act as the primary contact for prospective residents and families, guiding them through the admissions process in a busy skilled nursing setting in Massachusetts.

You will coordinate referrals, gather necessary information, obtain approvals, and collaborate with nursing, rehab, social services, dietary, and leadership to welcome new residents. Strong organizational and communication skills are essential.

Qualifications

• Three years of experience in long-term care.
• Knowledge of Medicare, Medicaid, OBRA and related regulations.
• Proficient with basic computer applications and accurate recordkeeping.
• Clear written and spoken English.

Responsabilités

• Serve as a primary contact for prospective residents and families; conduct tours and gather information for applications.
• Coordinate admission from referral through arrival with clinical liaisons, leadership and providers.
• Collect and review clinical and financial referral information; obtain approvals within 15–30 minutes.
• Obtain nursing management’s clinical approval before admission.
• Verify insurance coverage and payment sources with the Business Office; follow up as needed.
• Complete admission paperwork with residents or their representatives.
• Collaborate with nursing, rehab, social services, dietary and other departments to prepare for arrivals.
• Ensure preadmission screenings and OBRA/PASRR requirements are completed.
• Maintain admission records, advance directives, emergency contacts, and resident reps.
• Track referrals, update the database weekly, and report to the Administrator.
• Check in with residents and families; support satisfaction surveys and follow-up.
• Protect resident confidentiality and rights throughout admission.
• Perform related duties as assigned by the Administrator.
