Job summary

Alden Poplar Creek Rehabilitation and Health Care Center is seeking an Activity Aide to support the activity program and daily calendar for residents, focusing on quality of life and person-centered care.

You will work under the Activity Director to assess residents’ needs, coordinate daily activities, and document progress while communicating with families and staff. The role requires compassion, reliability, and ability to engage diverse residents.

Qualifications

• High school education or equivalent.
• At least 1 year experience in healthcare, volunteering, or related with disabled population.
• Current knowledge of Federal and State activity regulations.
• Able to read and write English.
• Bi-lingual abilities may be preferred by facility.
• Must have a valid IL driver’s license.
• Punctual and time efficient, especially with activity calendar.
• Empathy, compassion, understanding, enthusiasm, personable, energetic, positive and humorous when interacting with residents, families, and staff.
• Able to relate and work with ill, disabled, elderly, emotionally upset, and hostile individuals.
• Able to communicate effectively with the community, health professionals, and other providers.
• Understand and manage emotional needs of self and staff.

Responsibilities

• Provide feedback to Activity Director to monitor facility and program goals through a team approach.
• Lead a therapeutic, person-centered activity program to enhance residents’ quality of life.
• Follow all Activity Policies, Procedures and regulations.
• Know each resident’s background to tailor person-centered activity plans.
• Schedule daily activities reflecting residents’ choices and rights.
• Communicate residents’ language needs and use interpreters as required.
• Conduct and complete ongoing assessments across social, medical, cognitive, communication, spiritual and behavioral aspects.
• Document all required activity notes and maintain records.
• Update activity boards monthly with thematic ideas.
• Facilitate community involvement and invite external groups to events.
• Arrange resident outings and assist with transportation when needed.
• Follow staff schedules and collaborate with team for continuity of care.
• Maintain activity schedule structure and avoid cancellations without director approval.
• Keep equipment and supplies clean and restocked; notify when materials needed.
• Update calendars and boards daily.
• Perform other duties as assigned.
