Job summary

PACS is seeking an Activity Assistant to support the Activity Director in delivering a creative, interactive activities program aligned with regulatory standards and resident needs.

The role involves planning and conducting activities, coordinating transportation, and maintaining schedules and records. Strong reading, math, and communication skills are essential, along with the ability to work with diverse residents and families.

Qualifications

• High school diploma or equivalent.
• Preferably one year of experience in a long-term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Participate in planning and conducting individual, small and large group activities.
• Assist in communication between employees, residents, families, and public to meet residents' needs.
• Participate in community planning related to facility interests.
• Develop and maintain monthly activity calendar and attendance records.
• Assist in action plans to correct quality deficiencies as requested by Activities Director.
• Participate in discharge planning and development of activity care plans and resident assessments.
• Arrange transportation for residents for outings, appointments, or discharge.
• Develop monthly activity schedule including outings and in-room activities for bed-bound or isolated residents.
• Encourage participation in self-initiated activities and provide materials including braille or audio books.
• Assist with assessment documentation and quarterly progress notes.
• Maintain clean, orderly Activity Department.
