Addus HomeCare in Albuquerque, NM, is seeking a Full-Time Home Health Weekend Intake Coordinator (Remote) to cover Friday 8am–6pm, Saturday 8am–8pm, Sunday 8am–8pm, and Monday 8am–6pm. The role manages referrals from intake through completion, secures required documentation, and prepares complete referral packages for admission.

Qualifications

• High school diploma or equivalent; further education preferred.
• Experience in home health intake or healthcare coordination preferred.
• Knowledge of Medicare guidelines is a plus.
• Strong organizational and communication skills.

Responsibilities

• Manage assigned referrals from receipt through completion and readiness for scheduling.
• Review referrals for completeness, accuracy, and eligibility.
• Ensure all required documentation is obtained for a complete referral package.
• Verify that ordering MD signs required orders prior to admission.
• Provide welcome calls and explain services to patients/caregivers.
• Collaborate with scheduling teams to notify when referrals are ready for visits.
