Job summary

Visiting Nurse Association of Florida is seeking a Weekend Intake Coordinator to manage new patient referrals, conduct welcome calls, and coordinate with the scheduling team for seamless home health transitions. The role is based in Stuart, FL, with a full-time schedule Thursday–Monday, 8:00 AM–5:00 PM, including a 1-hour lunch.

The candidate should have strong communication skills, be detail-oriented, and comfortable working with electronic medical records.

Qualifications

• Experience in intake, home health, medical office, scheduling, or care coordination preferred.
• Experience with EMR systems.
• Compassionate and patient-centered approach.

Responsibilities

• Process and finalize new patient referrals in Homecare Homebase.
• Complete patient welcome calls and document coordination notes.
• Follow up on pending and hold patients, including hospital and facility updates.
• Complete pending and hold reports for assigned districts.
• Work with scheduling to ensure smooth transition into home health care.
• Provide clerical support to the Nursing Supervisor.
• Answer incoming calls and route to the appropriate resource.
