Qualifications

• 2-4 years of experience in healthcare settings, medical billing or coding.
• Experience in call center or customer service environments.
• Ability to work independently with a caring, proactive approach.
• Strong written and verbal communication skills.
• Proficiency navigating multiple systems (MS Office 365, clinical platforms, health plan systems).
• Technical proficiency to use digital tools and secure systems.

Responsibilities

• Initial review and triage of Care Teams tasks.
• Identify reasons for admission and apply intervention assessment tools.
• Screen patients to identify needed medical services.
• Refer to medical staff and coordinate services per benefit plan.
• Monitor non-targeted cases for discharge date and disposition.
• Identify and refer outlier cases to clinical staff and other programs.
• Use Med Compass and Aetna systems to manage member data.
• Support development and implementation of care plans.
• Coordinate care delivery under nurse/medical direction with providers.
• Promote internal and external communication to enhance medical management.
• Conduct non-medical research for open/closed cases.
• Provide support services to team members.
• Assist in researching and resolving claims payment issues.
• Support hospital care, case management and QA processes per standards.
